Home Care Packages
The Australian Government’s Home Care Package program supports older people to live independently. Having a Home Care Package is a great way to get support with one or more everyday tasks in your home. These tasks can be done with you or for you as required, making sure you have a choice over the services you need.
If you are eligible for a home care package, a subsidy is paid to an approved home care provider of your choice towards a package of care to meet your individual needs.
Home Care Package Levels
Level 1 – basic care needs
Level 2 – low-level care needs
Level 3 – intermediate care needs
Level 4 – high-level care needs.
Services Can Include
Services can be completed by sub-contractors such as cleaners, gardeners and skilled trades.
HART Services delivers home care packages on a Consumer Directed Care (CDC) basis. This means we will talk to you about your needs and goals and work with you to develop your individual care plan. You will have choice and control over what services are delivered, where and when they are delivered and who delivers them to you.
Our Client Care Manager will work with you to develop a care package plan and budget outlining how your package is funded and what services and care needs are included.
We will provide you with monthly income and expense statement so that you know how your budget is tracking and we will ensure that your package continues to meet your needs with ongoing monitoring and formal reviews.
Who is eligible for a home care package?
Once a package is approved you can choose a provider. HART Services is an approved aged care provider who will work closely with you to put together a care plan.
